Many of us have heard of some of the transmitter systems within our bodies, such as the sympathetic nervous system, which gives us the fight or flee response. Fewer have heard of the more recently discovered endocannabinoid system (ECS), which is surprising considering that the ECS is critical to almost every aspect of our functioning from moment to moment.

The ECS regulates and controls many of our most critical bodily functions such as learning and memory, emotional processing, sleep, temperature control, pain control, inflammatory and immune responses, and eating.

ECS is currently at the centre of international research and drug development.

What is ECS?

The ECS involves a vast network of chemical signals and cellular receptors that are densely distributed throughout our brain and body.

The “cannabinoid” receptors in the brain – CB1 receptors – outnumber many of the other types of receptors in the brain. They act like traffic cops to control the levels and activity of most of the other neurotransmitters. That’s the way they regulate things: by direct feedback, raising or lowering the activity of whatever system needs to be adjusted, whether it’s hunger, temperature or alertness.

To stimulate these receptors, our bodies produce molecules called endocannabinoids, which have a structural similarity to the molecules in the cannabis plant.

The first endocannabinoid discovered was called anandamide from the Sanskrit word ananda meaning bliss.

We all have tiny cannabis-like molecules that are all over our brains. The cannabis plant, which humans have been using for about 5,000 years, essentially acts as a ‘pirate’ of this ancient cellular mechanism.

A second type of cannabinoid receptor, the CB2 receptor, exists primarily in our immune tissues and is critical for controlling our immune function and plays a role in regulating intestinal inflammation, contraction and pain in inflammatory bowel conditions.

CB2 receptors are particularly exciting targets for drug development because they do not cause the blissful relationship with cannabis that stimulation of CB1 receptors (which is often an undesirable side effect) causes.

We know that ECS plays a critical role in learning and memory because of a lot of research work.

The most obvious observation is that one of the main side effects of high doses of recreational cannabis use is the temporary impairment of short-term memory.

There have also been some sophisticated studies on how people respond acutely to the administration of THC (the active ingredient in cannabis) and the ways in which this changes both their ability to remember short-term things and the patterns observed in the functional imaging of their brain.

According to popular author Michael Pollan in his best-selling book The Botany of Desire, cannabis is one of the plants that humans have cultivated or co-evolved with for thousands of years. This is in part, Pollan writes, because the act of forgetting plays a valuable role in our brain’s ability to function without being overloaded with data from our senses that we are constantly bombarded with. Pollan assumes that if we didn’t forget, we wouldn’t function, and cannabis helps us do that.

The role that ECS plays in forgetting also opens up opportunities for the treatment of PTSD, a condition in which there are unpleasant, disturbing memories that people cannot help but remember and which cause a whole syndrome of disturbing and dangerous symptoms associated with the pathological memory.

The cautionary tale of the drug rimonabant, a drug that blocks the CB1 receptor, is an interesting example of the central role that the ECS plays in so many critical functions. It was developed as an anti-obesity drug. The thought was that ECS controls hunger. We know this because, among other lines of evidence, cannabis gives you “the cooking”, so if you block the CB1 receptor it should cause weight loss. Rimonabant caused weight loss, quite successfully. But because ECS also regulates mood, it had to be taken off the market on an emergency basis because people taking it were starting to commit suicide. However, we can imagine a situation, as we better understand the complexity of ECS, where we may be able to create a weight loss drug that acts on those cannabinoid receptors that affect weight loss, but does not act on those receptors that control mood. Exploration of the ECS may lead to new drug discoveries.

The study of the ECS initially focused on efforts to understand (and demonize) an illicit drug, but new research has since blossomed into a much broader exploration into what is an amazingly complex and extensive system by which our bodies learn, feel, motivate and keep themselves in balance.

We are truly at the dawn of an era of ECS discovery and the development of new drugs that may help alleviate some of the most cruel diseases from which humans (and animals) suffer.
